This tool supports the Health Links program, which provides a new model for care in Ontario at the clinical level, in which all health care services in a community accessed by a patient are coordinated into a single plan. SHIIP provides the technology to enables a multi-agency care process by collecting patient demographic and clinical data from hospital databases.
